New Study Shows Popular Pain Relievers May Pose Risk
A new study comparing three popular arthritis drugs shows theyre all about equally safe — and equally dangerous for the heart.
Its good news for people who have to take pills every day for chronic pain, but the drugs are not completely safe. Ibuprofen, naproxen and the prescription drug celecoxib all can cause heart problems, especially when taken long term in high doses.
The findings only apply to people taking high doses over time they do not apply to people who take ibuprofen or naproxen for fevers, headache or the occasional aches and pains. But even occasional use should be limited, experts say.
My advice to the public is to take the lowest dose you can for the shortest period you can, said Dr. Steve Nissen, the heart expert at the Cleveland Clinic who ran the study.
The study was started because of a scandal going back to 2004, when the new arthritis drug Vioxx was pulled from the market when it was shown to cause heart attacks and other heart deaths.
It was a shocker because Vioxx, approved in 1999, was designed to be a more refined version of the drugs in the same class as aspirin and ibuprofen, which both can cause dangerous stomach bleeding.
Since then there has been a bit of suspicion surrounding other so-called COX-2 inhibitors, including celecoxib, sold generically and under the brand name Celebrex.
We gave everybody a drug to protect the stomach and it happened anyway.”
Its not completely clear why the drugs affect heart attack and stroke rates.

Is It Possible To Prevent Nsaid
NSAIDs are valuable medications for patients with arthritis and other inflammatory conditions. For patients who need long-term NSAID treatment, several steps can be taken to decrease NSAID-related ulcers and complications. The risk of ulcers and complications tends to be dose related. Therefore, the smallest effective dose of NSAIDs is taken to minimize the risk. NSAIDs might be selected that have less effect on the stomachâs production of prostaglandins. Some of these NSAIDs are called selective Cox-2 inhibitors. Cox-2 inhibitors block the Cox-2 enzyme that produces prostaglandins of inflammation without blocking the prostaglandin production of Cox-1 in the stomach. Currently in the United States, the only available selective Cox-2 inhibitor is celecoxib .
Taking NSAIDs with meals may minimize stomach upset with NSAIDs, but not ulcerations.
A synthetic prostaglandin, misoprostol , can be administered orally along with NSAIDs. Misoprostol has been shown to decrease NSAID-induced ulcers and their complications. The side effects of misoprostol include abdominal cramps and diarrhea. Misoprostol also is avoided in pregnant women because it can cause uterine muscle contractions and miscarriage. Standard doses of H2-blockers and proton pump inhibitors reduce the risk of NSAID-induced ulcers.
How And When To Take Naproxen
Always take your naproxen tablets with or just after a meal so you do not get an upset stomach.
As a general rule in adults, the dose to treat:
- diseases of joints is 500mg to 1,000mg a day in 1 or 2 doses
- muscle, bone disorders and painful periods is 500mg at first, then 250mg every 6 to 8 hours as required
- attacks of gout is 750mg, then 250mg every 8 hours until the attack has passed
Doses are usually lower for elderly people and people with heart, liver or kidney problems.
The doctor will use your child’s weight to work out the right dose.
If you get naproxen on prescription, the dose depends on the reason why you’re taking it, your age, how well your liver and kidneys work, and how well it helps your symptoms.
If you buy naproxen from a pharmacy for painful menstrual periods:
- on the first day take 2 tablets when the pain starts, then after 6 to 8 hours take 1 more tablet that day if you need to
- on the second and following days take 1 tablet every 6 to 8 hours if needed

A group of researchers from the an international network of researchers wanted to find out which medications can prevent the development of peptic ulcers. To do so, they looked for studies where NSAIDs were combined with PPIs, H2 blockers or misoprostol. The researchers found 41 suitable studies with a total of more than 21,000 participants.
The people in the studies were mainly men and women who had osteoarthritis or a rheumatic disease. All of them took one or several NSAIDs for at least three weeks and had a fairly high risk of getting a peptic ulcer. A number of the studies compared proton pump inhibitors, H2 blockers or misoprostol with a placebo or with each other. The participants were observed for up to one year. Regardless of whether they had symptoms, in almost all of the studies the participants had a gastroscopy to see whether they had an ulcer in their stomach or duodenum.

Special Warnings And Precautions
Problems with stomach ulcers and stomach bleeding can occur with any NSAID, and naproxen is no exception. Typically, these problems are tied to long-term use of the drug, but not alwaysshort-term use of naproxen or other NSAIDs can be problematic for some people.
Stomach ulcers and bleeding can occur without warning. Signs can include burning stomach pain, black stools, or vomiting. Call your doctor if you are experiencing these symptoms.
Liver damage can occur in people taking NSAIDs like naproxen. Warning signs include nausea, vomiting, fatigue, appetite loss, itching, yellowing of the skin or eyes, and dark urine.

How To Take Naprosyn
This oral medication can be taken as an immediate or extended-release tablet, suspension form, or even topically. If you are taking the extended-release Naprosyn tablets, swallow them whole. Do not crush, split, chew, or try to dissolve them.
You can take Naprosyn with food, milk, or antacids to help decrease the chances of an upset stomach. If you are taking antacids, avoid taking any with ingredients such as magnesium oxide or aluminum hydroxide. If you are going to take an antacid, the recommendation is to take one containing aluminum and magnesium hydroxide. This is preferred over others.
If you are taking the EC tablets, make sure you take the antacid and your Naprosyn dose at least two hours apart from each other.
As a rule for all NSAIDs, Naprosyn treatment should begin with the lowest effective dose and be used for the shortest possible duration. If your pain is severe enough, your provider may prescribe opioid medications as well. Examples of opioids include hydrocodone, oxycodone, and codeine.
For less severe pain, you can take Naprosyn with other pain relievers like Tylenol. However, it should not be taken together with other NSAIDs such as ibuprofen, aspirin, and celecoxib.
What Monitoring Might I Need
If you’ve been prescribed naproxen to take for long periods of time, your doctor might want you to have regular check-ups so that you can be monitored for any possible naproxen side effects. This might include routine blood tests to check your kidney function, liver function and levels of blood cells, particularly if you’re over 60.
