Emerging evidence from a number of studies suggests there is an association between long-term use of proton pump inhibitors and the development of gastric cancer. If needed, alternatives to PPIs are available.

Should I Stop Using Prilosec

PPIs like lansoprazole and omeprazole are available over the counter without a prescription. But long term use can be problematic.

The U.S. Food and Drug Administration says over-the-counter PPIs should only be taken for the 14-day course up to three times a year.

In some cases, medications called an H2 blocker like Pepcid or Tagamet may be appropriate. After a week of taking an H2 blocker instead of PPIs, the PPIs will be out of the system. It is then possible to switch to OTC antacids on an as-needed basis.

However, some people may still experience difficult symptoms after coming off PPIs and may need to go back on them at a higher dosage.

Before making any changes to medications or dosages, patients who are on PPIs should discuss with their healthcare provider whether they should continue taking the medication, or whether other options might be appropriate.

Is The Fda Going To Recall Nexium Prilosec Or Prevacid

Under federal law, drug manufacturers bear responsibility for the content of their warning labels, not the FDA. The FDA does has enforcement mechanisms, and the FDA sometimes forces drug manufacturers to add certain warnings, but the FDA doesnt have nearly enough resources to monitor every side effect for every drug. Federal law requires drug companies do that, and requires those same companies to add warnings whenever there is reasonable evidence of a causal association. Our lawyers believe there is reasonable evidence of a causal association and that the companies behind these antacids should have added a warning.

Gastric cancer isnt the only serious risk of drugs like Nexium and Prilosec. Some studies have tied the use of antacids in pregnancy to a higher rate of asthma in children, and others have found PPI users have a greater chance of developing dementia and an increased risk of death from all causes, particularly with long-term use and in patients who dont have gastrointestinal conditions.

The medical community has started to recognize how proton-pump inhibitors are over-prescribed. As several researchers noted in a recent article,

How Do Proton Pump Inhibitors Work

The stomach digests food with the aid of gastric acid, a mix of hydrochloric acid, sodium chloride and potassium chloride. This highly corrosive soup is produced when gastric cells in the stomach secrete the hormone gastrin, which signals parietal cells in the stomach to start pumping acid. PPIs shut down the proton pumps in the parietal cells, cutting off the production of acid. If you are blocking acid production by using a proton pump inhibitor, the body says, I dont have enough acid. Make more. So, the way the body reacts is for G-cells to produce more gastrin, Dr. Perre says. Too much gastrin may stimulate the growth of gastrointestinal tumors. Prolonged Use Of Antacids May Double Stomach Cancer Risk

Long-term intake of drugs commonly used to treat acid reflux conditions such as heartburn and ulcers may double the risk of developing stomach cancer, a study warns.

The risk associated with the use of proton pump inhibitors rose in tandem with the dose and duration of treatment following elimination of Helicobacter pylori, the bacteria implicated in the development of stomach cancer, said researchers from The University of Hong Kong.

Eliminating H pylori from the gut significantly lowers a person’s risk of developing stomach cancer.

However, a substantial proportion of those in whom treatment is successful still go on to develop the disease, the third leading cause of cancer death in the world.

Previous research has found an association between PPI use and heightened stomach cancer risk, but it was unable to factor in the potential role of H pylori itself, so undermining the strength of the findings.

The study published in the journal Gut compared the use of PPIs with another type of drug used to dampen down acid production called histamine H2 receptor antagonists in 63,397 adults treated with triple therapy – a combination of a PPI and two antibiotics to kill off H pylori over seven days.

They were subsequently monitored until they either developed stomach cancer, died, or the study ended, whichever came first. The average monitoring period lasted 7.5 years.

During this time, 3,271 people took PPIs for an average of nearly three years and 21,729 took H2 blockers.

Signs Of Serious Condition

Frequent heartburn could signal a chronic condition or gastroesophageal reflux disease , which afflicts more than 7 million Americans annually. Ignoring frequent symptoms could lead to complications such as strictures , ulcers, difficulty swallowing and even esophageal cancer.

“Taking antacids may mask a more serious problem,” said Qureshi, who is also chief of endoscopy at the Michael E. DeBakey VA Medical Center.

Antacids work by neutralizing the stomach acid that refluxes into the food pipe. In extreme cases, taking too many antacids, especially with milk, can cause kidney damage due to the amount of calcium intake from the pills on top of what is already found in your diet. There are more effective medications available, so talking with a doctor about solutions to the problem is important.

What To Do If You Have Stomach Cancer Or Ulcer Symptoms

  • If you have burning pain in your upper stomach that is relieved by eating or taking antacids, call a health-care professional for an appointment. Don’t assume you have an ulcer. Certain other conditions can cause similar symptoms.
  • If you vomit blood or have other signs of gastrointestinal bleeding, go to an emergency department right away. Peptic ulcers can cause massive bleeding, which requires blood transfusion or surgery.

What Is Acid Reflux

After you eat, the muscle at the end of your esophagus known as the esophageal sphincter ordinarily closes to keep food where it’s supposed to be: in your stomach. According to the National Institute of Health , gastroesophageal reflux, or GER, occurs when the sphincter becomes weak and allows your stomach contents to come back up. The result? That tell-tale burning sensation in the middle of the chest. When someone experiences heartburn on the regular, it can become a condition called gastroesophageal reflux disease .

Proton Pump Inhibitors: Do They Cause Cancer

Research suggests proton pump inhibitors, a type of antacid, could lead to stomach cancer, but some doctors question the study.

Antacids are an important treatment option for those with acid reflux. However, research in 2017 suggested that a type of antacid called proton pump inhibitors could lead to stomach cancer.

This study does not prove that proton pump inhibitors cause increased risk of stomach cancer, said Dr. Sueyi Lai, oncologist with Marshfield Clinic Health System. It suggests an associated increased risk, but it is just a link that needs to be looked at further.

Doctors typically use proton pump inhibitors, such as Prilosec or Prevacid, and two antibiotics to treat acid reflux. They can also use H2 blockers, such as Pepcid and Tagamet, as an antacid.

A bacterial infection called H. pylori in the stomach is what typically causes acid reflux.

This bacterial infection also can cause gastritis, gastric ulcers and stomach cancer. If untreated, the risk for stomach cancer and other complications grows.

The study shows there is a very tiny risk in a subgroup of patients that are already at an increased risk for stomach cancer, said Lai.

How Is Gerd Diagnosed

A physician can often make a GERD diagnosis based on patient symptoms, says Tsion Abdi, M.D., an assistant professor of medicine in the division of gastroenterology at Johns Hopkins School of Medicine in Baltimore. To confirm a diagnosis or check for complications, doctors may recommend further evaluation with upper endoscopy, motility testing such as an ambulatory acid probe test or imaging with a barium swallow, he adds. In a barium swallow test, you gulp a thick, chalky drink. As you swallow the barium, a radiologist takes pictures via x-rays or fluoroscopy to see how the barium moving through your mouth and throat.

What Are The Symptoms Of Insufficient Stomach Acid

What Are The Symptoms Of Stomach Cancer

Gastric cancer presents differently, Dr. Alexander says. The usual presentation is with nausea, weight loss, and upper abdominal pain. Symptoms of more advanced stomach cancer can include vomiting, stomach pain, weakness, frequent burping, jaundice, fluid buildup in the abdomen, and trouble swallowing. If youve got something down in the pit of the stomach that is present 24/7 or continual pain in the abdomen, these symptoms suggest gastric cancer.

What Causes Low Stomach Acid

Common factors that cause low stomach acid include stress, bacterial infection, poor diet, and nutritional deficiencies. An H. pylori infection, which is linked to stomach ulcers, is the most common cause of low stomach acid.

Hashimotos hypothyroidism can contribute to low stomach acid, as stomach function requires healthy thyroid function.

Pernicious anemia and deficiencies in zinc B12, magnesium, or chloride are other things to watch out for.
