A drug commonly used to treat acid reflux is linked to a more than doubled risk of developing stomach cancer, researchers have claimed.
Proton pump inhibitors reduce the amount of acid made by the stomach and are used to treat acid reflux and stomach ulcers.
A study published in the journal Gut identified an association between long-term use of the drug and a 2.4 times higher risk of developing stomach cancer. In the UK, there are more than 50m prescriptions for PPIs every year but they have been linked to side-effects and an increased risk of death.
A link between PPIs and a higher stomach cancer risk has previously been identified by academics but never in a study that first eliminates a type of bacteria suspected of fuelling the illnesss development.
Research by the University of Hong Kong and University College London found that after the Helicobacter pylori was removed, the risk of developing the disease still rose in line with the dose and duration of PPI treatment.
They compared the use of PPI against another drug which limits acid production known as H2 blockers in 63,397 adults. The participants selected had been treated with triple therapy, which combines PPI and antibiotics to kill off the H pylori bacteria over a week, between 2003 and 2012.

The good news is H2 blockers, another class of heartburn medications such as Pepcid and Tagamet , are safer alternative medications to the PPIs. The same study using H2 blocks as control medications to the PPIs showed that the stomach cancer risk of PPIs is 2-8 times higher than the H2 blockers.

Meta-analyses have shown that aspirin reduces gastric cancer risk without stratification according to H. pylori status., To address this issue, we conducted another study on the effect of aspirin specifically on H. pylori-eradicated subjects. Aspirin was shown to be associated with a lower gastric cancer risk among H. pylori-eradicated subjects , with a frequency, duration, and doseresponse relationship being observed. A subsequent post hoc analysis showed that the harmful effect of PPIs was higher among nonaspirin users, while PPIs were not associated with an increased risk of gastric cancer among aspirin users . The potential detrimental effect of PPIs on gastric cancer appears to be negated by aspirin use, although this observation necessitates validation by studies from other centers. Nevertheless, our study finding prompts the investigation of the role of other potential chemopreventive agents, such as metformin and statins, in reducing gastric cancer risk among PPI users should be further explored.

Strengths And Limitations Of This Study
This study has several strengths. First, to our knowledge, this is the largest study with the longest follow-up period conducted to date. Given the number of gastric events observed in our cohort, this study was sufficiently powered to address the long-term safety of PPIs and assess the risk among important subgroups, including by duration of use. Second, we restricted the cohort to new drug users, eliminating biases associated with the inclusion of prevalent users. Third, the comparator group consisted of patients prescribed H2RAs, an active comparator that likely minimised confounding by indication. Moreover, the use of propensity score-weighted methods ensured an excellent balance of all baseline confounders. Finally, our results remained highly consistent across several sensitivity analyses.
In summary, the results of this large real-world study suggest that patients newly treated with PPIs may be at an increased risk of gastric cancer compared with patients newly treated with H2RAs, although the absolute risk remains low. While PPIs have established clinical benefits when used according to evidence-based guidelines, this study highlights the need for physicians to regularly reassess the necessity of ongoing treatment. This is especially important in patients who are prescribed PPIs in the long term and for patients without an evidence-based indication for use.
